Our Social Services department is here to help you and your family, whether you’re an inpatient, outpatient or in transitional care. The main goal of our social workers is to see that you or your loved one is safe after being discharged. We connect patients with community services and programs.
We make a discharge plan for every patient who leaves the hospital based on our team’s evaluation. The team includes doctors, nurses, pharmacists, rehab therapists, dietitian, social worker, pharmacist and chaplain. Our social workers coordinate the plan. |
