Going Home
Planning for your hospital discharge
Your physician and St. Mary’s Hospital of Superior staff, together with you and
your family, will plan for specific aspects of your discharge. Such planning will
offer continued care to assist you at home or in another setting following the discharge.
You may need special supplies and equipment in your home or a referral to a public
health department or a home care agency within your community. It is entirely your
choice as to which company or agency you might wish to receive services from.
A social worker is available to assist you with making any necessary arrangements.
Call Social Services at ext. 5401, or ask your nurse to contact Social Services
for assistance.
We encourage you to ask any questions or express any concerns you or your family
may have about your return home.
Home health care
There are a variety of home health providers in the area from which to choose. Such
services offer a continuation of comprehensive care to you and your family after
discharge from the hospital. Services include nursing, rehabilitation therapy visits,
and home health aide services. Services are provided as determined by your needs
and are coordinated with you and your family, your physician, and the Medical Center
and Home Health staff.
Your insurance company,
Medicare or Medicaid, may pay for home care services. The
staff can discuss financial arrangements with you. To arrange for care within your
home, please contact your nurse or social worker.
Transportation
Usually a relative or a friend is available to take a patient home. However, if you need any assistance in making transportation arrangements, please ask your nurse
or social worker to help. A hospital attendant will take you to one of our hospital
entrances in a wheelchair for your safety and comfort.
Make arrangements for your transportation to arrive as early as possible. This will
allow you and your family to return home earlier in the day which will be easier
and less stressful. Remember to check your room before you leave for any belongings
and take home any valuables you may have placed in the hospital safe.
